Nurse Specialist

VNA Health Group is seeking a dedicated Registered Nurse (RN) to join our Hospice Team in Ocean County, New Jersey.
Job SummaryWe are looking for a compassionate and skilled RN to provide high-quality care to patients with life-threatening and life-limiting illnesses and their families.
Responsibilities- Assess and evaluate patient needs, developing an appropriate plan of care
- Identify teaching needs and plan strategies for individuals and families
- Monitor quality and appropriateness of services provided
- Collaborate with patient, family, and healthcare providers to provide care
- Document changes in patient condition and update care plans
- RN with Bachelor's degree in Nursing
- Certification in Hospice Palliative Care Nurse and Critical Care skills preferred
- 1+ year of supervised clinical experience as a registered nurse
