Job Title: Registered Nurse - Home Health

VNA Health Group is seeking a skilled Registered Nurse to join our Home Health team in Brielle, New Jersey.

About the Role:

We are looking for a compassionate and dedicated Registered Nurse to provide high-quality care to our patients in their homes. As a Registered Nurse on our Home Health team, you will be responsible for assessing patient needs, developing and implementing individualized care plans, and providing education and support to patients and their families.

Responsibilities:
  • Assess patient needs and develop individualized care plans
  • Provide direct patient care, including wound care, medication management, and vital sign monitoring
  • Collaborate with interdisciplinary team members to ensure comprehensive care
  • Provide education and support to patients and their families
  • Document patient progress and outcomes
Requirements:
  • Current RN licensure in New Jersey
  • 1+ year of experience in home health or hospice care
  • Excellent communication and interpersonal skills
  • Ability to work independently and as part of a team
