Job Summary

Alternate Solutions Health Network LLC is seeking a skilled Physical Therapist to provide high-quality patient care in a home-based setting.

Key Responsibilities:

  • Provide comprehensive physical therapy services to patients in their homes, as ordered by the attending physician.
  • Develop and implement individualized plans of care to promote patient recovery and independence.
  • Collaborate with interdisciplinary teams to ensure seamless care coordination and communication.
  • Conduct thorough assessments and evaluations to identify patient needs and develop effective treatment plans.
  • Utilize evidence-based practices and techniques to deliver high-quality patient care.
  • Document all aspects of patient care, including assessments, treatments, and progress notes, in accordance with agency protocol and Medicare/Federal guidelines.

Requirements:

  • Current licensure as a Physical Therapist in the state of employment.
  • Minimum of one year of experience as a Physical Therapist in an acute setting.
  • Home care experience preferred.
  • Ability to effectively communicate and create positive impressions with patients, families, physicians, and co-workers.
  • Knowledgeable on therapy best practices and ability to make appropriate physical therapy judgments.
  • Ability to identify situations and handle them with the best possible solution.
  • Detail-oriented and observant.
  • Disciplined style of work ethic with the ability to prioritize and be timely.
  • Valid driver's license and auto insurance with your name as a listed driver.
