Job Title: Advanced Practice Provider

AlignMed Partners is seeking a highly skilled Advanced Practice Provider to join our team at our Millville Center in Millville, NJ.

Job Summary:

The Advanced Practice Provider will provide primary and urgent care services to nursing center residents in collaboration with other medical providers in the center.

Responsibilities:
  • Provide high-quality patient care to residents in a long-term care setting
  • Collaborate with the Center Leadership Team to achieve AlignMed Partners' mission of improving the lives of those we touch through the delivery of high-quality healthcare and everyday compassion
  • Work collaboratively with the Medical Director and/or Attending Physician for patient-specific medical care
  • Participate in the development and implementation of care plans for residents
  • Conduct regular assessments and evaluations of residents' health status
  • Develop and implement individualized care plans to meet the unique needs of each resident
  • Collaborate with other healthcare professionals to ensure seamless care transitions and coordination
  • Stay up-to-date with the latest clinical guidelines and best practices in geriatric care
Requirements:
  • Graduate from an accredited School of Nursing program
  • Licensed as a registered nurse
  • Certified as a Nurse Practitioner by being a nursing graduate from an accredited Nurse Practitioner Program with a Master's Degree in Nursing and a concentration and specialty track of Older Adult and/or Family Nurse Practitioner
  • A minimum of two years of experience working with adult or geriatric populations
  • APRN with an unrestricted professional license and DEA, for each state in which they practice
