Key Responsibilities:
  • Provide personalized support and care to individuals with complex needs
  • Develop and implement personalized plans to promote independence and community involvement
  • Collaborate with a multidisciplinary team to ensure comprehensive care and support
  • Assist with daily living tasks, including personal hygiene and mobility
  • Support individuals with behavioral challenges and other complex needs

Requirements:
  • High School Diploma or equivalent
  • 6 months of experience in human services, direct care, or care coordination
  • Valid driver's license and reliable transportation
  • Ability to work with individuals with complex needs and behaviors
