Clinical Nurse Specialist

Maxim Healthcare Services is seeking a highly skilled Registered Nurse (RN) to provide exceptional patient care and support in a home health setting.
Responsibilities:- Assess and evaluate patient needs, developing personalized care plans to ensure optimal outcomes.
- Collaborate with healthcare teams to coordinate patient care, including medication management and treatment plans.
- Provide education and support to patients and their families, promoting health literacy and self-management skills.
- Conduct thorough assessments, identifying potential risks and developing strategies to mitigate them.
- Document patient progress, updating care plans as needed to reflect changing needs and circumstances.
- Current Registered Nurse (RN) license for the state in which the nurse practices.
- Current Health Certificate (per facility Registered Nurse (RN) contract or state regulation).
- Current PPD or Chest X-Ray.
- Current BLS card.
- One year prior Registered Nurse (RN) experience preferred.
- Pediatric care.
- G-tube management.
- Tracheostomy care.
- Ventilator management.
