Liberty Hospice is seeking an experienced Hospice & Palliative Care Nurse Practitioner to join our team.
Job Summary:
We are looking for a skilled nurse practitioner to implement palliative care program requirements, complete medical visits with patients, and coordinate care with the Palliative Care team. The ideal candidate will have a Master's degree in Nursing, current licensure, and certification as an Adult or Family Nurse Practitioner.
Responsibilities:
- Implement palliative care program requirements
- Complete medical visits with patients per guidelines and coordinate care with the Palliative Care team
- Conduct face-to-face encounters to provide re-certification information to the Medical Director/Associate Medical Director
- Provide clinical findings to the Primary care physician, and/or supervising Medical Director/Associate Medical Director
- Communicate with patients/families and all agency personnel
Requirements:
- Master's degree in Nursing required, with current licensure in the state of practice
- Certification as an Adult or Family Nurse Practitioner
- Experience in Hospice or Palliative Care nursing preferred
- Knowledge of the framework, function, and organization of a hospice care program
- Excellent communication and organization skills
