This role requires various nurse licensure and certification based on role and grade level. The function identifies, coordinates, or provides appropriate levels of care under the direct supervision of an RN or MD. It's responsible for clinical operations and medical management activities across the continuum of care (assessing, planning, implementing, coordinating, monitoring and evaluating). This includes case management, coordination of care, and medical management consulting.

The estimated salary for this position is $83,500 - $115,000 per year, depending on experience and qualifications. The work schedule is Monday-Friday, business hours for three days then 11am - 8pm for two days a week.

Key Responsibilities:
  • Make outbound calls and receive inbound calls to assess members' current health status
  • Make referrals to outside sources
  • Identify gaps or barriers in treatment plans
  • Provide patient education to assist with self-management
  • Provide a complete continuum of quality care through close communication with members via in-person or on-phone interaction
Requirements:
  • Undergraduate degree or equivalent experience
  • Current, unrestricted RN license in the state of employment
  • 3+ years of experience in a hospital, acute care or direct care setting
  • Ability to support a Hybrid work schedule, including consumer centric hours (11a - 8p CST) two days a week
  • Ability to type and have the ability to navigate a Windows based environment
