Job Summary

Kaiser Permanente is seeking a skilled Clinical Social Work Professional to join our team in Lutherville-Timonium, Maryland.

Job Description

This is a full-time position that requires a Master's degree in Social Work (MSW) and a minimum of one year of clinical social work experience in a healthcare setting. The ideal candidate will be licensed as a Licensed Independent Social Worker (LISW) in the District of Columbia, a Licensed Master Social Worker (LMSW) in Maryland, and a Licensed Master's Social Worker (LMSW) in Virginia.

Key Responsibilities
  • Collaborate with the healthcare team to coordinate appropriate services for members, ensuring cost-effectiveness and consistency with their plan of care.
  • Conduct comprehensive psychosocial assessments to evaluate patient goals, social support systems, resources, health status, functional limitations, psychological status, environmental factors, and response to treatment.
  • Develop and monitor a plan of care to promote the member's optimal level of functioning and enhance their quality of life.
  • Identify, facilitate, and advocate for appropriate organizational and community resources to meet the plan of care, ensuring timely and cost-effective implementation.
  • Ensure consistent and reliable documentation of case management activities in compliance with organization and department standards.
  • Analyze patient and program outcomes to identify improvements in program, quality, and cost-effectiveness of case management activities.
  • Provide supportive counseling and education to members, families, and caregivers, as well as members of the healthcare team, health plan staff, and the community.
Requirements
  • Master's degree in Social Work (MSW) required.
  • Minimum one year of clinical social work experience in a healthcare setting required.
  • Licensed Independent Social Worker (LISW) in the District of Columbia, Licensed Master Social Worker (LMSW) in Maryland, and Licensed Master's Social Worker (LMSW) in Virginia required within six months of hire.
Preferred Qualifications
  • Experience with computer software programs in a Windows environment preferred.
  • Knowledge of community systems and resources in the defined service area preferred.
  • Knowledge of regulatory issues for the Mid-Atlantic area preferred.
