Job Title: Family Nurse Practitioner

At South Central Family Health Center, we are seeking a highly skilled and compassionate Family Nurse Practitioner to join our team of healthcare professionals. As a Family Nurse Practitioner, you will play a vital role in providing high-quality, patient-centered care to individuals and families in our community.

Job Summary:

The Family Nurse Practitioner will provide a wide range of healthcare services, including health promotion and disease prevention, direct care and counseling, and coordination of health services. You will work closely with patients, families, and other healthcare professionals to develop and implement individualized care plans that address the unique needs of each patient.

Responsibilities:
  • Provide primary care and physical examinations, including Pap tests and other routine screenings
  • Conduct laboratory tests and interpret results
  • Write and dispense prescriptions, and perform office procedures within the scope of expertise and available equipment
  • Counsel patients and families on health-related issues, including lifestyle and development concerns
  • Coordinate health promotional activities, such as providing printed information, routine testing, and health education
  • Manage overall patient care, including follow-up and referrals to other medical services and specialists as needed
Qualifications:
  • Master's degree in a related field preferred
  • Knowledge of and adherence to the American Nurses Association Code of Ethics for Nurses
  • Minimum of 6 years of RN experience related to family health
  • Recent experience in the use of Electronic Medical Record documentation
  • Bilingual Spanish/English required
  • Valid California Registered Nursing License
  • Licensed as a Nurse Practitioner in the State of California
  • Current DEA and BLS certifications
