Position Title: Housing First Blended Case Manager
POSITION SUMMARY: The Housing First Blended Case Manager plays a crucial role in collaborating with clients to create and implement a goal-oriented strategy that addresses their health (both physical and emotional), social, and environmental requirements identified through comprehensive assessments. This position is dedicated to empowering clients to optimize their use of personal and community resources, connecting them with necessary services while ensuring that these resources are provided in the least restrictive manner possible. The case manager is responsible for facilitating timely access to essential services, ensuring appropriate care levels, and maintaining continuity of care.
Qualifications: A Bachelor’s degree in Social Work from an accredited institution recognized by the Council on Social Work Education, or a Bachelor’s degree in a closely related field within health or human services from an accredited college or university.
Experience: Candidates should demonstrate relevant skills and competencies through previous employment, internships, or volunteer experiences that showcase their aptitude for the role.
Additional Credentials: Specific credentials or documented training may be required for certain assignments within this classification. Details regarding necessary qualifications will be outlined in the hiring documentation.
KEY RESPONSIBILITIES:
- Provide Information & Referral assistance within the assigned program to ensure the public receives accurate information about available services and is effectively connected to appropriate community resources.
- Conduct comprehensive assessments for each assigned client to identify and prioritize their biopsychosocial needs, developing a Service Plan to address these needs systematically.
- Assist clients in executing their Service Plans by linking them to various service systems, facilitating access to necessary services, coordinating multiple service providers, and advocating on their behalf when needed.
- Deliver crisis intervention services for both assigned and non-assigned clients during emergency situations.
- Coordinate Discharge, Aftercare, and Follow-up Plans and services for clients.
- Provide supportive counseling tailored to clients' needs within the scope of the specific program and based on prior professional training.
- Establish and maintain professional relationships with colleagues and community service providers to enhance service delivery.
- Engage in professional development through supervision and continuing education to enhance social service knowledge and skills.
- Ensure adherence to client privacy and confidentiality rights as per agency policies.
- Perform administrative tasks and other duties as assigned.
