About the Role

We are seeking a dedicated and compassionate Foster Care Case Manager to join our team at Presbyterian Children's Homes and Services. As a Foster Care Case Manager, you will play a vital role in providing safety and support to children and families in foster care.

Job Summary

The Foster Care Case Manager is responsible for facilitating Family Support Team meetings, developing individualized care plans, coordinating services across settings, and documenting support activities. You will promote effective utilization of support services to achieve permanency for children and families.

Main Responsibilities
  • Fulfills all contractual and agency requirements, ensuring seamless delivery of services to children and their families.
  • Maintains a caseload of 12 to 15 children, depending on program capacity and agency needs, with temporary adjustments possible due to extended absences or position vacancies.
  • Provides full case management services to children and their families in the Missouri foster and adoptive care system from assignment until case closure or transfer.
  • Diligently documents all activities and maintains data entry in FACES and CSWIS in a timely and accurate manner.
  • Engages in regular face-to-face contact with assigned children and their families, as well as telephone communication with family support providers, to review progress and address concerns.
  • Evaluates and reports the impact of services to team members and judiciary, attends court hearings, schedules and coordinates Family Support Team meetings, and participates in an FCCM consortium on-call schedule.
Requirements and Qualifications

To be successful in this role, you should possess:

  • A bachelor's degree in social work, psychology, counseling, education, criminal justice, or a related human services field; a master's degree is preferred.
  • At least two years of relevant case management experience.
  • Excellent verbal and written communication skills, along with strong representation and rapport-building abilities.
  • A flexible schedule, with ability to work outside regular hours when needed, and plan, arrange, and provide crisis intervention services as necessary.
  • The ability to operate computer software and navigate multiple data entry systems.
Benefits

This role offers a competitive salary range of $43,000 to $49,000 per year, as well as benefits such as medical, dental, and vision insurance options, paid time off, and a generous retirement plan after one year of service and 1,000 hours worked, with a 200% match up to a 3% contribution.
