About the Role

We are seeking a highly motivated and dedicated Foster Care Case Manager to join our team at Presbyterian Children's Homes and Services. As a Foster Care Case Manager, you will play a vital role in supporting children and families in need by providing Christ-centered care and services.

Job Summary

The Foster Care Case Manager is responsible for ensuring the safety and well-being of children and families in foster care. This includes facilitating Family Support Team meetings, developing individualized care plans, coordinating services across various settings, and documenting support activities.

You will work closely with children, parents, and families to promote effective utilization of support services, achieve permanency, and provide crisis intervention services as needed.

Key Responsibilities
  • Case Management: Maintain a caseload of 12-15 children, providing full case management services from assignment to case closure or transfer.
  • Documentation: Document all activities and maintain accurate data entry in FACES and CSWIS systems.
  • Communication: Have face-to-face contact with assigned children and families monthly, with telephone contact with providers regularly.
  • Evaluation: Evaluate and report on service impact to team members and judiciary, attending court hearings and scheduling FST meetings.
Requirements

To be successful in this role, you must possess a bachelor's degree in social work, psychology, counseling, education, criminal justice, or a related human services field. A master's degree is preferred, along with a minimum of two years of relevant case management experience.

Key skills include strong verbal and written communication, ability to represent the agency well, and build positive relationships with community stakeholders. You must also have excellent time management, organization, planning, and assertive communication skills.
