Job Title: Clinical Nurse Liaison

 

About the Opportunity:

As a Clinical Nurse Liaison at Community Healthcare of Texas, you will play a vital role in bridging the transition between hospital care and hospice services. This position requires strong communication and interpersonal skills to develop long-term relationships with hospitals and families.

Key Responsibilities:

  • Referral Evaluation and Response: Timely evaluation of patient appropriateness for hospice services and responsive communication with referral sources.
  • Patient-Centered Care: Meeting with families/patients to discuss goals and explaining hospice services.
  • Collaboration and Communication: Working closely with hospital staff, DME, medications, hospice physicians, and hospital staff to ensure smooth patient discharges.
  • Relationship Building: Educating hospital staff, following up on pending patients, and building relationships through daily rounds.
  • Documentation and Reporting: Maintaining accurate and thorough documentation of hospice eligibility within required timeframes.
  • Strategic Partnership Development: Fostering strong relationships with hospitals to increase referrals and establish ourselves as the preferred hospice provider.

Requirements and Qualifications:

  • Licensure and Education: Registered RN in the State of Texas or LMSW required; nursing experience or medical training essential.
  • Marketing Experience: Proven marketing experience required; 1-3 years of healthcare marketing experience preferred.
  • Hospice Experience: Strongly preferred.
  • Technical Skills: Proficient computer skills required.

Compensation and Benefits:

  • Annual Salary Range: $62,000 - $72,000
