Medical Care Coordinator

We are seeking a Medical Care Coordinator to join our team at Family & Community Services, Inc. in Kent, OH. This is a full-time position with a salary of $45,000-$60,000 per year.
Job Description:The Medical Care Coordinator will play a critical role in delivering quality medical care within the context of a federally qualified community health center. The ideal candidate will function under the supervision and guidance of the Health Center Administrator and participate in the multi-disciplinary process of providing care and treatment to patients.
The Medical Care Coordinator will be responsible for conducting pre-visit planning, assisting providers with patient office visits, preparing and administering medications and/or procedures as directed by the medical provider, and providing counseling and health education to patients.
The selected candidate will work effectively within a team environment, exchanging honest and constructive feedback, supporting team goals, encouraging other team members, and collaborating with patient care team members to consolidate work, solve problems, create efficiencies, and reduce costs.
The Medical Care Coordinator must demonstrate flexibility/adaptability in meeting environmental constraints and demands, assume accountability for demonstrating behaviors consistent with API's written policies and procedures, and follow Health Department guidelines regarding communicable diseases.
This position requires excellent customer service skills with patients and their families, other staff, providers, vendors, and the public. The successful candidate will also be able to lift, carry, and store clinical equipment and supplies within the center as directed.
Requirements:- Bachelor's degree in nursing or related field
- Current licensure as a Licensed Practical Nurse (LPN)
- 1+ years of experience in a clinical setting
- Excellent communication and interpersonal skills
