About the Role:
As a Care Coordinator, you will be responsible for coordinating the Resident Assessment Instrument (RAI) process to ensure accurate and timely completion of resident assessments.
Key Responsibilities:
- Coordinate RAI process to ensure accurate and timely completion of resident assessments
- Ensure assessments accurately reflect the physical, mental, and psycho-social status of each resident
- Communicate effectively with other members of the interdisciplinary team
- Follow all Washington Square Healthcare policies and procedures
Requirements:
Must be an LPN or RN with excellent communication and interpersonal skills.
