RN MDS/Care Plan Coordinator

Villa at City Center is seeking a skilled and dedicated Care Plan Coordinator RN to join our team. As a key member of our healthcare team, you will play a vital role in coordinating the development and implementation of resident care plans, ensuring that each resident receives the highest quality care.
Key Responsibilities:
- Coordinate the development and implementation of resident care plans in accordance with state and federal regulations and facility policies.
- Communicate each resident's care plan to facility personnel via the written care plan.
- Provide ongoing assessment and needed care plan changes as warranted to meet each resident's needs.
- Ensure that direct care personnel accurately identify and document resident level of functioning within required time frames.
- Maintain an accurate care plan calendar to ensure that care plans are completed per federal and state requirements.
- Coordinate all IDT elements of the care plan process, including outside service provider care.
- Follow all Villa policies and procedures and perform duties as defined by the state nurse practice act.
Requirements:
- Possesses a current RN license to practice in the state.
- Skilled nursing facility experience as an MDS nurse is preferred.
- Strong computer-related skills, including proficiency in Microsoft Office Word and an email system.
- Strong oral and written communication skills.
- Strong organizational and project management skills.
- Ability to work with all levels of employees.
- Ability to read, write, speak, and understand English.
Environmental and Physical Requirements:
- Walking, reaching, bending, lifting, extended sitting, grasping, fine hand coordination, pushing, and pulling are required.
- Limited potential for exposure to environmental hazards. Understanding and adherence to company safety standards and protocols are required.
- Meet all health requirements as needed.
