We are seeking a skilled Clinical Nutrition Specialist to join our team at D-H Lebanon-MHMH. As a key member of our clinical nutrition services team, you will play a vital role in evaluating, assessing, and planning nutrition care to meet the complex needs of our patients.
Responsibilities- Assess patient nutritional status using documented medical information and established nutritional standards.
- Counsel patients and/or families by identifying nutrition-related problems, developing plans of instruction and nutritional care, and monitoring results.
- Identify inpatients at nutritional risk and document recommendations.
- Participate in physicians' rounds, discharge planning rounds, and conferences with nutritional implications.
- Develop and implement departmental policies, procedures, nutritional standards, patient educational material, and QI projects.
- Conduct education programs for patients, community groups, and staff.
- Bachelor's Degree from an accredited program in nutritional sciences.
- Prior specialty experience preferred.
- Current NH Dietitian license required.
