About the Role

We are seeking a skilled Speech Language Pathologist to join our Home Health team. As a key member of our care team, you will work with patients in their homes to evaluate, direct, and provide speech/language pathology services.

Responsibilities
  • Evaluate and assess patients' speech, language, and swallowing disorders
  • Develop and implement individualized treatment plans to address patients' needs
  • Provide instruction and training to patients and their families on the use of alternative communication systems
  • Collaborate with other healthcare professionals to ensure comprehensive care
  • Document patient progress and maintain accurate records
Requirements
  • Master's degree in Speech-Language Pathology or related field
  • Current certification in Speech-Language Pathology (CCC-SLP)
  • Minimum of six months experience as a speech therapist/speech-language pathologist
  • Home Health experience a plus
  • Current and unrestricted license
  • Current CPR certification
  • Good organizational and communication skills
