About the Role

We are seeking a dedicated and compassionate Registered Nurse Case Manager to join our team at Ohio Valley Hospice. As a key member of our interdisciplinary team, you will play a vital role in providing high-quality care to our patients and their families.

Responsibilities
  • Conduct comprehensive assessments of patients and families to identify their unique needs and develop personalized care plans.
  • Collaborate with the interdisciplinary team to ensure seamless care coordination and effective communication.
  • Administer medication and treatment as prescribed by the physician, while also providing emotional support and counseling to patients and their families.
  • Maintain accurate and detailed clinical notes, while also participating in interdisciplinary group meetings to ensure comprehensive care.
  • Ensure compliance with all state and federal legal and regulatory requirements, while also staying up-to-date with the latest developments in hospice care.
Requirements
  • Graduate of an accredited nursing school with current licensure to practice in the state of operation.
  • At least one year of nursing experience, with three or more years preferred.
  • Experience in hospice or a similar setting is highly desirable.
  • Knowledge of the hospice philosophy of care and a commitment to clinical and documentation excellence.
