As a Hospice Registered Nurse Case Manager at BrightSpring Health Services, you will play a vital role in providing exceptional care to patients and their families. Your compassion, expertise, and dedication will make a meaningful impact in the lives of those we serve.

About the Role

Our Hospice Registered Nurse Case Managers are the heart of our organization. In this role, you will support your patients by providing direct care and coordinating with the interdisciplinary team to ensure that both patients and their families have an optimal care experience. You will be responsible for completing initial and ongoing comprehensive assessments of patients and families, initiating patient plans of care, and reevaluating as necessary. You will also utilize health assessment data to determine nursing diagnoses, administer medication and treatment as prescribed by the physician, and counsel the patient and family in meeting nursing and related needs. Additionally, you will complete and maintain accurate clinical notes, communicate with the physician, and participate in interdisciplinary group meetings to meet the medical and emotional needs of the patient and family/caregivers. You will ensure compliance with all state and federal legal and regulatory requirements and perform additional responsibilities as assigned.

Requirements

To be successful in this role, you will need to have a graduate degree from an accredited nursing school with current licensure to practice in the state of operation. You will also need at least one year of nursing experience, with three or more years preferred. Experience in hospice or a similar setting is also preferred. You will need to have knowledge of the hospice philosophy of care and a commitment to clinical and documentation excellence.
