As a key member of our clinical team, you will be responsible for developing and implementing individualized care plans, educating patients and their families, and coordinating with healthcare professionals to ensure seamless care transitions.
Key Responsibilities- Develop and implement comprehensive care plans in accordance with physician orders and patient needs
- Provide education and support to patients and their families on disease management, medication adherence, and self-care techniques
- Collaborate with primary caregivers to ensure optimal functioning and safety of patients
- Coordinate with healthcare professionals to ensure timely and effective care transitions
- Provide skilled nursing care in patients' homes, including wound care, infusion therapy, and catheter management
- Active RN license in the state of application
- Valid CPR certification
- 1 year of experience in a healthcare setting, preferably in home health care
- Medicare skilled nursing experience
- Basic understanding of OASIS
