Join Our Team as a Home Health LPN

At CenterWell Home Health, we're dedicated to providing personalized, comprehensive home care for patients managing chronic conditions or recovering from injury, illness, surgery, or hospitalization. As a Home Health LPN, you'll play a vital role in our care teams, working alongside nurses, physical therapists, occupational therapists, speech-language pathologists, home health aides, and medical social workers to help patients rehabilitate, recover, and regain their independence.

Key Responsibilities:
  • Provide professional nursing services under the direction of a RN in compliance with state laws and regulations, healthcare standards, and CenterWell Home Health policies and procedures.
  • Deliver skilled nursing interventions in the treatment of patients' illnesses, rehabilitative needs, and preventative care, utilizing a holistic approach.
  • Collaborate with the RN Supervisor to develop and implement individualized patient plans of care, ensuring timely and effective interventions.
  • Communicate effectively with patients, physicians, clinical managers, and other healthcare team members regarding patient needs and status changes.
  • Maintain accurate and detailed documentation of patient care, adhering to company and facility requirements.
Requirements:
  • Graduate of an accredited Licensed Practical Nursing Program or accredited School of Vocational Nursing.
  • Current nursing license in the practicing state.
  • Valid driver's license, auto insurance, and reliable transportation.
  • Current CPR certification.
  • Two years of experience as an LPN/LVN in a clinical setting, preferably in home health or hospice care.
