Job Summary

We are seeking a skilled RN Home Care Nurse to join our team at the East Boston Neighborhood Health Center. As a key member of our care delivery team, you will be responsible for providing high-quality nursing care to patients in their homes.

About the Role
  • Assess patient needs through obtaining medical history, performing tests, and making observations.
  • Develop and implement individualized care plans in collaboration with healthcare providers.
  • Provide education and guidance to patients and their families on health-related topics.
  • Administer medications and treatments as ordered by healthcare providers.
Requirements

To be successful in this role, you will need:

  • A baccalaureate degree in nursing or equivalent experience.
  • A current Massachusetts license in nursing.
  • Current BLS certification required.
  • A minimum of two years' experience in ambulatory, home health, hospital, or sub-acute settings.
