Position Overview

The RN Unit Care Coordinator plays a crucial role in overseeing, implementing, coordinating, and managing patient care through effective communication with patients, families, nursing personnel, and other stakeholders within the unit, ensuring compliance with all relevant laws, regulations, and Life Care standards.

Qualifications and Licensure
  • Associate's or Bachelor's degree in Nursing
  • Current licensure as a Registered Nurse (RN) in the applicable State, maintaining good standing throughout employment
  • Preferred: Minimum of one (1) year experience in geriatric nursing
  • CPR certification required upon hire or to be obtained during orientation, with ongoing maintenance throughout employment
Key Responsibilities
  • Possess advanced knowledge in nursing practice
  • Exercise independent judgment when necessary
  • Familiarity with practices, procedures, and regulations governing post-acute care facilities
  • Implement and interpret departmental programs, goals, objectives, policies, and procedures
  • Demonstrate proficiency in all competency areas, including patient rights, safety, and sanitation
  • Maintain professional relationships with all associates and vendors
  • Safeguard confidentiality of proprietary and sensitive information
  • Adhere to company policies, including those related to harassment and compliance
  • Exhibit integrity and professionalism by following Life Care's Code of Conduct and completing mandatory compliance training
Essential Duties
  • Effectively lead the daily operations of unit nurses and CNAs, providing guidance on the floor
  • Document patient care accurately and in a timely manner
  • Manage and coordinate patient care services to help patients achieve or maintain optimal physical, mental, and psychosocial well-being
  • Prepare and administer medications as prescribed by physicians
  • Respond to emergency situations with a leadership approach to ensure patient and staff safety
  • Coordinate comprehensive patient care plans and services
  • Deliver exceptional customer service and maintain a positive demeanor towards patients
  • Assist in patient evacuation procedures when necessary
  • Demonstrate reliable attendance and punctuality
  • Utilize reasoning skills and sound judgment in all situations
  • Communicate effectively and collaborate within an interdisciplinary team
  • Engage in physical activities such as sitting, standing, bending, lifting, pushing, pulling, walking, and reaching throughout working hours
  • Read, write, speak, and comprehend the English language
