Job Overview

Terry Reilly Health Services offers a rewarding career opportunity as a Clinical Care Coordinator in our community health setting.

The estimated salary for this position is $65,000 - $85,000 per year, depending on experience.

Responsibilities
  • Provide patient care either in person or virtually, ensuring support for clinicians and fostering teamwork and quality patient care.
  • Implement and enforce protocols, policies, and procedures in accordance with a Patient Centered Medical Home.
  • Respond to crisis situations/critical patients, taking a lead role in maintaining a safe environment.
Essential Duties
  1. Assess and provide patient care responsive to patient needs, providing education and supportive services consistent with the care provided in a comprehensive Patient Centered Medical Home care delivery system.
  2. Support patient flow, intervening in elevated patient care needs to ensure a safe efficient patient flow and utilization of care team resources.
  3. Provide direct patient care, performing clinical interventions, patient education, generating all necessary EHR documentation, and completing support duties in a timely, professional, and efficient manner.
  4. Complete medical screening, medication administration, lab services, medication refill, and in-basket support, and any other standing orders as available.
Minimum Qualifications
  • Current Idaho Paramedic license
  • Current BLS certification
  • 1-year paramedic experience
  • Strong problem-solving, time management, and computer skills
Preferred Qualifications
  • 2+ years of clinical experience in a physical or virtual clinical care environments
  • Bachelor's degree in health science or related degree
  • Experience in a community health clinic environment or community paramedic setting working with a wide variety of patient populations
