Acute Inpatient Medical Care Coordinator

We are seeking a dedicated Licensed Master Social Worker (LMSW) or Licensed Clinical Social Worker (LCSW) to join our team as an Acute Inpatient Medical Care Coordinator.
About the RoleIn this role, you will provide advocacy, education, assessments, and interventions to patients and families in the inpatient hospital setting to achieve an optimal level of care.
You will also coordinate with medical staff and outside organizations to establish specific treatment plans and facilitate transition planning to achieve quality clinical outcomes.
This position will work primarily in the Emergency Department, providing quality care for ED patients and their families, and supporting ED clinical staff.
Key Responsibilities- Provide advocacy, education, assessments, and interventions to patients and families
- Coordinate with medical staff and outside organizations to establish treatment plans
- Facilitate transition planning to achieve quality clinical outcomes
- Work primarily in the Emergency Department, providing quality care for ED patients and their families
To be successful in this role, you will need:
- Licensed Master Social Worker (LMSW) required as defined by your primary work state (Idaho or Oregon), Licensed Clinical Social Worker (LCSW) preferred
- All colleagues must provide licensure or proof of application in process for the secondary state within 6 months from hire date
- Master's Degree in Social Work required
- Two years of experience in hospital setting or medical social work setting preferred
- Experience in case management, addiction services, mental health, or population health management desired

Salary InformationThe estimated salary for this role is $65,000 - $80,000 per year, depending on experience.
