We are seeking a dedicated Care Coordinator to join our team at the Idaho Department of Juvenile Corrections. Located in beautiful Nampa, Idaho, this role offers a unique opportunity to make a positive impact on the lives of juveniles.

About the Role

This position provides essential healthcare services and support to juveniles in our facility. As a Care Coordinator, you will be responsible for evaluating, treating, and referring medical needs, dispensing medication, arranging for ancillary services, maintaining accurate records, and educating juveniles and staff on health issues and disease prevention.

Key Responsibilities:
  • Provide comprehensive nursing care, health education, and charting as per the Nurse Practice Act.
  • Administer prescribed medications and monitor for adverse reactions.
  • Conduct interviews with juveniles to gather health information and provide guidance on basic health needs.
  • Offer health education to juveniles and staff on various health topics and disease prevention strategies.
Requirements

To be successful in this role, you must:

  • Hold a valid Licensed Practical Nurse (LPN) license from the state of Idaho or be eligible under the Nurse Licensure Compact.
  • Be willing to work in a juvenile correctional facility environment.
  • Meet federal PREA requirements.
