Job Overview

We are seeking a dedicated and compassionate Individual Support Coordinator to join our team at Hope Network. As an Individual Support Coordinator, you will work closely with individuals and their families to create personalized support plans, promoting independence and community participation.

Key Responsibilities
  • Conduct initial assessments and develop individualized support plans
  • Coordinate and deliver support services, including transportation, meal preparation, and household maintenance
  • Monitor and report any changes in an individual's health or well-being
  • Collaborate with healthcare professionals to ensure seamless care transitions
  • Document individual progress and care provided, ensuring accurate and timely information
Requirements

To be successful in this role, you will need:

  • A high school diploma or equivalent
  • A valid State of Michigan driver's license
  • The ability to lift 50 pounds
  • Certification in CPR/First Aid is preferred but not required
  • Basic computer skills and experience working with electronic medical records
Compensation and Benefits

We offer a competitive salary of $48,000 - $70,000 per year, depending on experience, plus benefits including paid time off, retirement savings plan, and opportunities for professional growth and development.
