Clinical Care Navigator

CareCentrix is seeking a skilled Clinical Care Navigator to join our team. As a Clinical Care Navigator, you will play a vital role in shaping the future of healthcare by expertly managing resource allocation, utilization review, and patient care coordination.
Key ResponsibilitiesAs a Clinical Care Navigator, your primary responsibilities will include:
- Determining optimal care settings for patients post-hospital discharge through collaboration with hospital discharge planners, case managers, hospitalists, and CareCentrix Medical Directors.
- Managing transitions of assigned Post-Acute Care patients from acute care settings to SNF, IRF, LTACH, or home settings utilizing face-to-face and/or telephonic outreach.
- Coordinating orders for home health and other post-acute care services.
- Communicating customer service and provider issues to supervisors for logging and resolution.
- Engaging and coaching patients/caregivers on progress toward goals, safe transitions to the next care site, and readmission avoidance resources.
- Documenting all interactions, problems, goals, and interventions to meet documentation guidelines.
- Working closely with home health agencies and post-acute providers to ensure smooth transitions to home or facilities with appropriate services.
To be successful as a Clinical Care Navigator, you must possess:
- A current and unrestricted Registered Nurse license.
- A minimum of five years RN experience, preferably with a geriatric population.
- Expertise in Utilization Management and knowledge of URAC & NCQA standards.
- Broad knowledge of healthcare delivery/managed care regulations, contract terms/stipulations, prior utilization management/case management experience, and governmental home health agency regulations.
- Negotiation, communication, influencing, problem-solving, and decision-making skills.
- High-level clinical knowledge, communication, customer service, and problem-solving skills, as well as the ability to effectively interact with all levels of management and a highly diverse clientele.
- Excellent communication, organizational, interpersonal skills, and the ability to effectively manage and prioritize tasks.
