Homeless Healthcare APRN

Cornell Scott-Hill Health Center seeks an experienced APRN to join our infectious disease team. As a Homeless Healthcare APRN, you will provide direct patient care to individuals experiencing unsheltered homelessness, conducting street outreach and offering medical services at day centers, soup kitchens, and other areas of need.
Responsibilities- Perform comprehensive medical evaluations, educate patients on treatment plans, and maintain confidentiality while ensuring compliance with agency, state, and HIPAA laws.
- Assess and interpret medical histories, perform physical examinations, identify problems, formulate treatment plans, provide education and referrals as needed, and evaluate interventions' effectiveness.
- Order laboratory tests, interpret results, and report communicable diseases in a timely manner to the relevant Health Department.
- Prescribe medications and implement standardized evaluations or assessments with multidisciplinary teams when necessary.
- Maintain accurate and timely documentation, obtain insurance authorizations, and participate in continuing medical education activities to enhance professionalism and quality of care.
- Master's degree in Nursing; valid CT APRN and RN license required.
- Minimum 2-4 years' experience working with the homeless population, including managing HIV/AIDS treatment and prevention.
- American Academy of HIV Specialist certification preferred, along with Suboxone DATA X waiver.
$83,000 - $110,000 per year, depending on experience, plus benefits package, including above-market salary, licensure reimbursement, and generous paid time off.
