Job Summary

We are seeking a skilled LPN to join our team as a Home Health Care Specialist. As a key member of our team, you will visit patients in their homes and provide nursing care under the direction of RNs and in compliance with physicians' orders.

Responsibilities
  • Provide nursing care to patients in their homes, under the direction of RNs and in compliance with physicians' orders.
  • Develop and implement individualized care plans to meet the unique needs of each patient.
  • Conduct thorough assessments of patients' physical and emotional needs, and document findings in a timely and accurate manner.
  • Collaborate with RNs and other healthcare professionals to ensure seamless care coordination and communication.
  • Participate in ongoing education and training to stay current on best practices and industry developments.
Qualifications
  • Current licensure as a Licensed Practical Nurse in the state of employment.
  • Valid state driver's license and automobile liability insurance.
  • Dependable transportation and ability to drive in all types of weather conditions.
  • Ability to work independently and as part of a team.
  • Excellent communication and interpersonal skills.
