Job Title: Home Care Private Duty RN

At Adara Home Health, we are seeking a compassionate and skilled Registered Nurse to join our team as a Home Care Private Duty RN. As a key member of our interdisciplinary team, you will provide high-quality, patient-centered care to our clients in the comfort of their own homes.

Job Summary:

The Home Care Private Duty RN will be responsible for furnishing comprehensive client care as assigned by the Case Manager, in accordance with the Nurse Practice Act, the basic principles of practical nursing, and the policies and procedures of Adara Home Health. This role requires strong interpersonal communication and teaching skills, as well as the ability to work independently and as part of a team.

Responsibilities:
  • Plan, provide, and evaluate nursing care in accordance with established policies and procedures.
  • Administer medications and treatments as ordered by the physician and delegated by the Case Manager.
  • Initiate rehabilitative nursing procedure techniques as appropriate.
  • Communicate effectively with all members of the interdisciplinary team.
  • Maintain accurate clinical records and prepare clinical and progress notes.
  • Assist clients in arranging medical appointments and contacts with community resources.
  • Maintain a safe client environment and identify and report any accidents, injuries, or suspected vulnerable client abuse, neglect, or financial exploitation.
Requirements:
  • Current licensure as an RN or LPN in the state of Minnesota.
  • At least one year of experience in a healthcare setting, with acute hospital or home care experience preferred.
  • Ability to make appropriate nursing judgments.
  • Strong interpersonal communication and teaching skills.
  • Strong organizational skills and the ability to work independently.
  • Excellent written and oral communication skills.
  • Proof of negative mantoux or documentation of negative chest x-ray.
  • Current CPR certification highly recommended.
