About the Role:

We are seeking a skilled Physical Therapist to join our team at Home Health Care, Inc. As a Physical Therapist, you will provide high-quality physical therapy services to clients in their homes, helping them achieve their goals and maintain their independence.

Responsibilities:
  • Provide physical therapy services to clients in their homes, including evaluations, initial and ongoing assessments, and development of treatment plans.
  • Collaborate with other clinicians, including Physical Therapist Assistants, to ensure comprehensive care for clients.
  • Communicate effectively with clients, families, and healthcare teams to ensure seamless care coordination.
  • Document client progress and outcomes in accordance with agency policies and procedures.
Requirements:
  • Current Physical Therapist license to practice in the State of Minnesota.
  • Experience in home care setting, including OASIS documentation.
  • Demonstrated ability to work autonomously and as part of a team.
  • Valid Driver's License and reliable transportation.
