Join Our Team as a Weekend Skilled Home Visit Nurse

Become a part of our caring community and help us put health first. As a Weekend Skilled Home Visit Nurse, you will play a vital role in providing skilled care and admissions to Home Health during weekends.

The schedule is flexible, including options like Thursday to Sunday, Friday to Monday, or Friday to Sunday. This allows you to have a better work-life balance and make the most of your weekends.

About the Role
  • Provide admission, case management, and follow-up skilled nursing visits for home health
  • Confer with physician in developing the initial plan of treatment based on physician's orders and initial patient assessment
  • Implement patient care plan in conjunction with patient and family to assist them in achieving optimal resolution of needs/problems
  • Coordinate/oversee/supervise the work of Home Health Aides, Certified Home Health Aides and Personal Care Workers
Requirements
  • Diploma, Associate or Bachelor Degree in Nursing
  • Minimum of one year nursing experience in Skilled Home Visits
  • OASIS proficiency Required
  • HCHB EMR experience Highly preferred
  • Current and unrestricted Registered Nurse licensure
  • Current CPR certification
  • Strong organizational and communication skills
