Dallas County is seeking a skilled Community Outreach Coordinator to provide support to clients in various medical, social, educational, and other social services programs. As a key member of our team, you will work under direct supervision to assist staff with locating, identifying, and contacting clients, providing medical or social services screenings, investigating, monitoring, and conducting follow-ups, and educating clients on available resources.

Key Responsibilities

  • Assist staff in providing outreach services to clients, patients, or the community in need of medical, social, education, financial, and other social services.
  • Assist staff with medical screenings, transportation, contact investigations, and follow-ups, documenting performance and status of services and providing progress reports and feedback to the supervisor.
  • Reinforce and provide educational information, literature, and documentation to clients, patients, or the community.
  • Interact with social services organizations to maintain established networks within the community.

Requirements

  • Graduation from an accredited high school or GED program.
  • Knowledge of standard software applications.
  • Ability to effectively communicate, both verbally and in writing, and establish and maintain effective working relationships with employees, departments, and the general public.
  • Valid Texas Driver's License, with a good driving record, and reliable transportation (depending on the location).
