Key Responsibilities- Case Management: Provide ongoing monitoring and support to individuals in the Assisted Outpatient Treatment (AOT) program, ensuring compliance with court orders and developing personalized treatment plans.
- Intake Assessments: Conduct thorough assessments to identify individual needs and develop effective treatment strategies.
- Service Coordination: Link individuals with appropriate treatment modalities, including public entitlements and benefits, to ensure seamless care coordination.
- Communication: Maintain open communication with the AOT Director, AOT Psychiatrist, and treatment team to ensure informed decision-making and effective care delivery.
- Transportation and Support: Provide transportation, support counseling, and crisis intervention services as needed to support individuals in their recovery journey.
- Documentation: Maintain accurate and detailed progress notes to track individual progress and service delivery.
- Team Collaboration: Participate in staff scheduling, training, and orientation to ensure a cohesive and effective team environment.
- Recovery Groups: Facilitate recovery groups and work with individuals to develop and implement WRAP plans.
- Therapeutic Relationships: Establish and maintain ongoing therapeutic relationships with individuals, adapting to their unique needs and circumstances.
- Service Delivery: Monitor and adjust service delivery to meet the evolving needs of program participants.
- Master's Degree: Possess a Master's degree in social work, psychology, counseling, or a related field.
- Experience: Demonstrate experience working with individuals with severe and persistent mental illness.
- Flexibility: Be able to work a flexible schedule, including evenings, weekends, and holidays.
- Valid Driver's License: Possess a valid driver's license in good standing.
