Job Title: Case Manager
Job Summary:
EAC Network is seeking a skilled Case Manager to join our team. As a Case Manager, you will be responsible for providing direct support to incapacitated adults, leveraging technology and innovative solutions to manage client care effectively.
Key Responsibilities:
- Initiate client services within 72 business hours of assignment, gathering necessary information and developing a comprehensive service plan.
- Provide ongoing case management services, including home visits, telephone reassurance, advocacy, and guidance to clients dealing with issues such as substance abuse, mental health challenges, housing instability, and financial difficulties.
- Transport and escort clients to appointments for emergency resources or to complete essential applications, ensuring their access to required services.
- Participate in regular case conferencing sessions for collaborative case review and decision-making with the team to ensure comprehensive client support and effective service delivery.
- Maintain accurate case records, documenting key interactions, progress, and any adjustments to the service plan.
Requirements:
- Bachelor's Degree in social or behavioral sciences (preferred but not required if you have relevant experience).
- At least two years of relevant work experience in social services, case management, or a related field.
- Strong proficiency in Microsoft Office, databases, and innovative tech solutions for managing cases.
- Excellent communication skills (both verbal and written) and the ability to connect with diverse communities.
- Valid NY State Driver's License and access to a vehicle for transporting clients when needed.
- Passion for social justice, advocacy, and helping vulnerable populations.
