Job Description
The Patient Engagement Specialist will work with patients, families, and healthcare providers to identify and address barriers to care. They will provide education and support to help patients navigate the healthcare system and make informed decisions about their care.
Required Skills and Qualifications
- Bachelor's degree in social sciences, health education, or related field of study
- Four years of experience in health education, public health, social work, and/or case management may be considered as a substitute for the Bachelor's degree
- English/Spanish fluency required
