We are seeking a highly skilled and compassionate Mental Health Case Manager to join our Adult Services team at Johnson County Kansas. As a key member of our team, you will work closely with adults who experience severe mental illness, providing community-based services and support to help them achieve their goals and improve their overall well-being.
Key Responsibilities- Conduct thorough assessments of individuals' background and circumstances to identify needs and determine service eligibility.
- Develop and implement personalized plans to address the needs of individuals, including networking with other community agencies to research and coordinate resources and services.
- Provide a range of services for a designated caseload of individuals, including transportation, skill development, and support groups.
- Document services provided, evaluate progress, and review the quality and effectiveness of services.
- Bachelor's degree in Human Services or a related field.
- Valid driver's license with an acceptable driving record.
- One year of related work experience is preferred.
