Integrated Targeted Case Manager

OVERVIEW OF PRIMARY DUTIES & RESPONSIBILITIES:

1. The individual in this role will support designated clients in accessing essential resources, including medical, social, educational, and other necessary supportive services. This position will be under the direct supervision of the assigned Case Management Supervisor.

2. These activities are aimed at achieving specific outcomes that benefit the individuals served and should align with the priorities outlined below in addressing their needs;

3. Mobilizing and utilizing the natural supports available to the individual through family, friends, neighbors, etc., when their needs cannot be fulfilled through these channels;

4. Coordinating and leveraging resources available within the local community and generic service providers. When the individual's needs cannot be met through these means, then

5. Securing specialized services through accessible mental health treatment and rehabilitation facilities. This includes service providers who focus on various disability groups, such as the Office of Vocational Rehabilitation.

CORE FUNCTIONS:

  1. Approximately 85% of the case manager's time will be dedicated to the following client and collateral activities.
  2. Evaluation of the client's history and current life circumstances with their input to formulate a service plan.
  3. Advocacy efforts to enhance services, reduce stigma, resolve issues, etc.
  4. Provision of supportive counseling to clients, their families, and other caregivers.
  5. Building informal support networks.
  6. Assisting clients in identifying, accessing, and effectively utilizing community resources to meet their daily living requirements.
  7. Referral and connection with suitable services as outlined in the treatment/service plan.
  8. A minimum of 15% of the case manager's time will be allocated to the following types of meetings and documentation activities;
  9. Recording client/collateral interactions.
  10. Monitoring service/treatment and service planning meetings.
  11. Engaging in individual and/or group supervisory meetings.
  12. Participating in treatment and service planning meetings.
  13. Attending staff or approved training sessions.
  14. Completing client assessment batteries and state client outcome forms and surveys to evaluate the impact of the County's Comprehensive Case Management Plan.
  15. At least 50% of the case manager's time will be spent outside the office in direct client or collateral interactions.
  16. Case Managers will have on-call responsibilities 24/7 and must be reachable by their supervisor via cell phone within a 15-minute response time to provide emergency/crisis support. Additionally, on-call Case Managers are required to make face-to-face contact with clients and the Crisis Response Center in the event of a potential hospitalization within 1.25 hours of the initial call.

ADDITIONAL DUTIES:

1. Undertakes other responsibilities and special projects as assigned.

QUALIFICATIONS & REQUIREMENTS:

  1. Bachelor's or Master's Degree in Human Services or a related field, and;
  2. Must be a Registered Nurse or;
  3. A High School Diploma with 12 semester credit hours in the human services or related field, along with two (2) years of experience, including one (1) year in direct client contact, or;
  4. A High School Diploma with five (5) years of mental health care experience in public or private human services, with prior employment as an intensive case manager before specified criteria, or;
  5. Any equivalent combination of training and experience as approved by the Agency and OHM guidelines.
