The OpportunityWe are seeking a skilled Primary Care Provider to join our team in San Diego. As a Primary Care Provider, you will be responsible for delivering care to our patients in their homes, working closely with our care navigators to maximize your time and energy. You will develop and execute detailed care plans for each patient, work with our Medical Director to identify and address emerging issues, and provide leadership to our clinical team.
Responsibilities- Provide onsite primary care services to older adults living in senior living communities
- Develop and execute detailed care plans for each patient
- Work with Medical Director to identify and address emerging issues
- Provide leadership to our clinical team
- Collaborate with care navigators to maximize your time and energy
- Board-Eligible Nurse Practitioner - preferably with geriatrics experience
- Comfortable with a fast-paced and changing environment
- Willingness to provide onsite clinical care
- Desire to work with complex care patients and support the building of a holistic care model
- Strong orientation towards collaboration and working closely with a multidisciplinary team
- Competitive salary range of $140,000 to $180,000 plus equity and benefits
