Job SummaryWe are seeking a compassionate and skilled Pediatric Nurse/Home Health Care RN to join our team. As a member of our team, you will provide one-on-one care to pediatric patients in their homes, utilizing your clinical expertise to create a safe and nurturing environment.
Responsibilities- Provide skilled nursing care to pediatric patients in their homes, including medication management, wound care, and vital sign monitoring.
- Develop and implement individualized care plans to meet the unique needs of each patient.
- Collaborate with healthcare teams to ensure seamless care transitions and optimal patient outcomes.
- Conduct thorough assessments and evaluations to identify patient needs and develop effective care strategies.
- Document patient progress and communicate with families and healthcare teams to ensure informed decision-making.
