Job Summary

We are seeking a highly skilled and compassionate Medical Staff Coordinator to join our team at East House. In this role, you will be responsible for coordinating referrals with OASAS and OMH residential programs, ensuring that potential clients meet the criteria for treatment-based housing.

Key Responsibilities

  • Assess individuals with diverse psychiatric and/or chemical dependency diagnoses
  • Provide coordination of referrals with OASAS and OMH residential programs
  • Coordinate Admission Safety Reviews for clients meeting risk criteria
  • Coordinate placement in appropriate units for accepted admissions
  • Provide outreach and information to potential referral sources about East House services
  • Report on referral status, trends, and service utilization to agency staff and community service providers
  • Maintain documentation to meet defined standards by regulators, funders, and East House policies

Requirements

  • Bachelor's degree in a human service-related field
  • Two years' paid human service experience or equivalent
  • Basic computer literacy and Microsoft Word skills
  • Experience using multiple database programs

Working Conditions

  • Traditional office or household environment
  • Frequent travel in Monroe County and surrounding counties
