Cadia Healthcare is a leading provider of skilled nursing and rehabilitation services. We are seeking a highly skilled and experienced Lead Registered Nurse Assessment Coordinator (RNAC) and RN/LPN MDS Coordinator to join our team.

Job Summary

The Lead RNAC/MDS Coordinator is responsible for leading the MDS team in accurate and timely completion of MDS assessments and coordination of the RAI process. This role requires strong leadership and communication skills, as well as the ability to work effectively with an interdisciplinary team.

Key Responsibilities
  • Coordinate the RAI process, including Minimum Data Set (MDS) care area assessment process and care plan development, implementation, and evaluation.
  • Oversee the baseline care plan and coordinate the completion of the comprehensive care plan according to regulatory requirements.
  • Determine skilled level of care for Medicare residents and procure required Medicare-specific documentation.
  • Coordinate care with case managers for residents utilizing managed care, health maintenance organizations (HMOs), commercial insurance, and alternative payment models.
  • Serve as a member of the quality assessment and assurance (QAA) and/or the quality assurance and performance improvement (QAPI) committees.
Requirements
  • Must possess a current, unencumbered active license to practice as a RN (Lead)/LPN in this state or compact state for the Lead position.
  • Minimum of 2 years MDS experience required.
  • Must know current state and federal regulations, including Sections of Appendix PP 483.20 Resident Assessments and 483.21 Comprehensive Resident-Centered Care Plans and Medicare and Medicaid Eligibility and Entitlement.
