We are seeking a compassionate and motivated Registered Nurse to join our team as a Wound Care Coordinator - RN. As a key member of our nursing center, you will lead and integrate a fully developed Skin Health and Wound management program, collaborating with nursing center management to deliver high-quality patient care.
Key Responsibilities- Lead and integrate a comprehensive Skin Health and Wound management program, ensuring adherence to national guidelines and wound care standards.
- Support central supply as the clinical expert for skin, wound, and incontinent products, promoting evidence-based practice.
- Oversee durable medical equipment related to wound prevention and management, ensuring optimal patient outcomes.
- Collaborate with the interprofessional team to develop and implement wound care plans, promoting a patient-centered approach.
- Provide education and training to nursing center staff on skin health and wound care best practices, ensuring a culture of excellence.
- Must possess a RN license (MPT/DPT with leadership review/approval).
- Must complete Skin Health Team Lead orientation training and Wound Basics within 90 days of hire.
- Commitment to attend monthly support calls is required.
- Must have national accredited wound certification (CWS, CWCN, WCC, CWON, WOC, AWCC) or be committed to complete Advanced Wound Training and national accredited Wound Certification within 1 year of hire.
- Knowledge and experience with clinical charting, incident reporting, and investigation response is required.
- Must have a strong knowledge base of acceptable standards of care for nurses, providers, therapists, dietitians, and others on the nursing center team.
- Experience in team building and demonstrating respect for the interprofessional team in wound management is required.
- Must have knowledge of national wound guidelines and wound products.
- Knowledge of nursing home regulations and survey process is required.
- Strong Microsoft Word, Excel, Database Management, PowerPoint skills are required.
