Position Overview:

Key Responsibilities:
  • Deliver primary care services in a post-acute setting for both short-term and long-term patients.
  • Manage chronic conditions, diagnose medical issues, and implement comprehensive care plans.
  • Order and interpret laboratory results to guide treatment decisions.
  • Prescribe and monitor medications effectively.
  • Collaborate with physicians and coordinate care with other healthcare professionals.
  • Educate patients and their families on managing chronic and age-related health challenges.
  • Foster relationships with patients, their families, and facility staff to enhance treatment outcomes.
  • Encourage and promote healthy lifestyle modifications.

Requirements:
  • Experience in post-acute or primary care is advantageous but not mandatory.
  • New graduates are encouraged to apply.
  • Possession of an active and unrestricted California NP or PA license or in the process of obtaining one.
  • Active and unrestricted California DEA license or in the process of obtaining one.
Compensation and Benefits:
Our compensation model is based on productivity (Fee for Service), allowing you to maximize your earning potential. The expected salary range is between $100,000 and $150,000 annually. This full-time position is eligible for a comprehensive benefits package, including:
