Job Description:

The Salvation Army USA Central Territory is seeking a dedicated and compassionate Community Support Specialist to join our team. As a key member of our Emergency Community Support Program, you will play a vital role in empowering individuals to achieve their highest level of independence and reach their maximum potential.

Job Summary:

As a Community Support Specialist, you will provide intensive case management services using a strength-based, person-centered approach. Your primary goal will be to promote recovery, facilitate meaningful participation in the community, resolve crisis situations, and prevent the need for higher levels of care.

Key Responsibilities:
  1. Participate in discharge planning from higher levels of care and establish a therapeutic relationship with clients.
  2. Provide pre-discharge transition services from psychiatric hospitals, including teaching daily living skills, scheduling appointments, transportation to appointments, and assistance with housing search as needed.
  3. Complete comprehensive assessments during initial meetings and develop individualized service plans within 5 days of admission. Collaborate with clients and support systems to develop and implement comprehensive service plans within 14 days of admission.
  4. Closely collaborate with clients, family members, and other service providers to ensure seamless care coordination.
  5. Provide education and support to clients and their families, developing and maintaining formal and informal support systems as needed.
  6. Respond promptly to crisis situations and develop safety and aftercare plans within 3 days of intake.
  7. Collaborate with clients on wellness recovery action plans.
  8. Provide advocacy and support, accompanying clients in the community to assess readiness for discharge.
  9. Assist clients in accessing benefits, such as Social Security Insurance, housing vouchers, food stamps, and Medicaid, and provide referrals to community-based behavioral health services.
  10. Participate in staff meetings, file reviews, case consultations, performance evaluations, and quality improvement initiatives.
  11. Provide on-call coverage, responding to client needs and addressing situations as necessary.
  12. Complete ongoing training and professional development, including Emergency Protective Custody training, Board of Mental Health training, and Wellness Recovery Action Plan training.
Requirements:

To be successful in this role, you will need:

  • A high school diploma or equivalent, with at least one of the following: two years of lived experience, two years of direct care experience in a human service field, two years of training in a human service field, or a bachelor's degree in a human service field.
  • Excellent communication and interpersonal skills, with the ability to work effectively with clients, families, and service providers.
  • Knowledge of Microsoft Office, including Word, Excel, and email software, as well as the ability to access payroll and personal data via a web-based system.
  • Administrative skills, including data entry, attention to detail, and filing.
  • Demonstrated ability to handle confidential information and maintain effective working relationships.
  • Sensitivity to the cultural and socioeconomic characteristics of our service population.
