Nutrition Care Specialist

Responsibilities- Assist patients in making informed diet and nutrition choices to improve their quality of life
- Conduct comprehensive nutrition assessments for new and at-risk patients
- Collaborate with the interdisciplinary team to develop and implement individualized care plans
- Identify specific interventions necessary to meet nutritional needs based on evidence-based guidelines and best practices
- Provide education and support to patients and their families on renal diet and fluid management
- Registered Dietitian Nutritionist with Commission on Dietetic Registration
- Minimum one year of professional work experience in clinical nutrition as a registered dietitian
- Previous renal care experience preferred but not required
