Senior Care Advocate Opportunity

We are seeking a dedicated and compassionate individual to join our skilled memory care community as a Senior Care Advocate. This role is crucial to enhancing the lives of our residents and providing person-centered care in a positive environment.

Key Responsibilities:
  • Support Resident Well-being: Collaborate with residents to achieve their highest level of well-being, addressing their physical, emotional, and social needs.
  • Conduct Assessments: Perform pre-admission assessments, memory screenings, and daily evaluations to ensure accurate care planning.
  • Orient and Support: Welcome new residents and their families, providing ongoing support and guidance throughout their stay.
  • Documentation and Planning: Complete timely and accurate documentation, and develop care plans for residents with severe memory loss.
  • Interdisciplinary Collaboration: Work with the interdisciplinary team to monitor residents and make recommendations for their care.
  • Life Goal Support: Assist residents in achieving their life goals and promoting their independence.
  • Memory Support Group: Co-facilitate a memory support group to provide a safe and supportive environment for residents and their families.
Requirements:
  • Education and Experience: Bachelor's degree in Social Work or a related field, with LSW or LCSW preferred. Experience working with older adults and persons living with dementia is a plus.
  • Interpersonal and Communication Skills: Excellent interpersonal and communication skills, with the ability to conduct oneself professionally with sensitivity and compassion.
  • Technical Skills: Strong computer and time management skills, with the ability to multitask and manage multiple deadlines.
